Social Prescribing is a person-centered, community-based approach that connects seniors with non-medical resources and activities to improve their overall wellness. Our program recognizes that health extends beyond medical care to include social connections, meaningful activities, and community support.
How It Works
Our Seniors Community Connector works with you through a simple 5-step process:
- Receive Referral – Accept referrals from healthcare providers, community partners, or self-referrals
- Needs Assessment – Understand your unique situation and challenges
- Resource Matching – Connect you with appropriate community resources and services
- Co-Develop Wellness Plan – Create a personalized plan together based on your goals and interests
- Follow Up – Provide ongoing support to ensure your plan is working for you
Who Can Participate?
- Residing in the South Vancouver area
- Age 65+ (flexible)
High Priority Participants:
- Low and moderate-income individuals
- Living alone or experiencing social isolation
- Facing moderate or significant health challenges
- Recently discharged from hospital and needing community support
